This was a template freebie which is no longer available. You can create this two ways. First, is by a program called Shape Collage. You can download it for free, but it will have "shape collage" somewhere on the page, unless you purchase the upgrade. The more you use it, the bigger the words become.
Second, is to just place all the photos you want to use onto the page and start arranging them until you get a pattern that pleases you. That will take longer, but then you'll be able to crop the photos and put them where you want.
